How to Enroll at SMS 2021-2022

Welcome to St. Mary School!

PLEASE BE SURE TO READ THIS ENTIRE DOCUMENT IN ORDER TO SEE ALL ITEMS NEEDED FOR REGISTRATION AS WELL AS ADDITIONAL INFORMATION REGARDING TUITION, TRANSFER GRANTS AND TUITION ASSISTANCE.

Applications for admissions are being accepted based on current enrollment capacity for the 2021-2022 school year.

In an effort to continue with our enrollment process for the 2021-2022 school year, St. Mary School has implemented the following registration information and procedures to help families interested in
applying for enrollment at St. Mary School to complete this process as early and as easily as possible.

All of our registration information packets are now available on-line below. They are grouped per grade level. (Kindergarten, Grades 1-7, 3 year old preschool and 4 year old preschool).  Under each group will be all of the forms needed for registration that you will need to return to us in addition to the required information listed below under each grade category ( ex: Birth certificate, baptismal, First Penance, First Communion immunization records, report cards, standardized test scores etc. ).

In order to comply with present social distancing directives, we ask if you could please print out these forms, fill them out, and then send them to the Advancement Office via email. You can scan them or take a photo of them and send them back through email.  We ask that you please include all of the other required paperwork as well.

All forms and paperwork should be emailed to Jackie Kern at Advancementoffice@smarys.org. 

In order to assure we receive all of your completed information, we ask if you could please send all of your forms and paperwork at the same time in the same email.

Transfer Grants: 

Transfer grants are offered to those students enrolling in grades 1-7 who are coming from public school, home school, charter school or out-of-diocese school. Only, if you qualify for this grant based on the school you are transferring from, Please fill out the form for the transfer grant and send it with the required forms and paperwork. This is a 2-year grant.

If you qualify for the grant you cannot apply for Tuition Assistance until year 3 of enrollment at St. Mary School.

Tuition Refund Policy 2021-2022

The 2020-2021 Tuition Refund Policy is listed under Admissions. As part of the registration process each family is required to sign the form indicating you received this policy. In this case, the Tuition Refund Policy is available for you to read and review via our website only.

Please send this signed form indicating that you received information that the policy is available to read and review and where it is found, back also as part of the registration packet via email.

FACTS Tuition:

All families are to be placed into FACTS Tuition Management for tuition to be automatically withdrawn from your account beginning in July and going through April. In order to do this, you will need to join FACTs secured online site and include your banking information for the account you will be using. In order to do this, you will need to go to our website, www.smarys.org and under LINKS you will see FACTS Tuition. Open this link and it will show you how to create an account. If you are a a current family, and enrolling and additional students, we will add that student to your current account.

FACTS Tuition Assistance:

Tuition assistance is still available. In order to apply for Tuition Assistance, go to our website www.smarys.org and under Links you will see FACTS Tuition Assistance where you can apply directly online through FACTS.

Registration Forms: Please click on the grade you are interested in, print out the forms, fill them out, scan them and email them to Jackie Kern at advancementoffice@smarys.org along with the additional required items listed below:

Additional Information and Items Needed for Registration
For incoming Preschool, Kindergarten AND
those NEW students to SMS who will be entering 1st-7th grade

KINDERGARTEN

(Child must be five years of age on or before September 30th)
Registration Fee – $100.00 (Non-Refundable)
Birth Certificate – Original and Copy
Baptismal Certificate – Original and Copy

Copy of latest Immunization Records ( we also require an updated immunization record if done after registration and prior to October 1st and after Kindergarten booster shots are given)

Flu shots are recommended but are not required for Kindergarten students.  If your student receives a flu shot you do not need to submit a doctor’s note.

Mileage to school from home for transportation form

In addition to the required forms and items needed for registration, your child will also need to have a Kindergarten screening. These Kindergarten screenings will be held in March or April. You will receive further information on the screening.

The $ 100.00 Registration fee may be mailed to St. Mary School at the following address:

You can also drop the registration fee off at the school’s Main Office on Carroll Ave. and place it in the drop box located outside the main doors.

If you are a current family and registering a new student you can send in the Registration Fee with your child: Attention Main Office.

Please be sure to include your child’s name and grade for September 2021 and indicate it is the registration fee.

Entering GRADES  1-7

Birth Certificate – Original and Copy
Baptismal Certificate – Original and Copy
Penance Certificate – Original and Copy (If applicable)
Communion Certificate – Original and Copy (If applicable)
Confirmation Certificate – Original and Copy (If applicable)
Copy of Latest Required Immunization Records – Students entering 6th Grade will need 1 Tdap or Adacel and 1 meningococcal Proof of these shots should be submitted no later than the first day of school.

Flu shots are recommended but are not required for grades 1-8.  If your student receives a flu shot you do not need to submit a doctor’s note.

Copies of latest (and 2 years prior if applicable) Report Cards, Standardized Tests, and Child Study Team Reports for our school files

Mileage from your home to St. Mary School for transportation form

The requirement for a 2-day visit for incoming 1st-7th grade students is currently on hold until further notice. We know the benefit of this experience is important; however, we are unable to do offer this at this time. Should we be able to have our incoming new students spend 2 days with us during the school day, you will receive further information from St. Mary School.

Registration is also dependent upon the availability of space.

3 and 4 Year old Preschool

4 year olds must be 4 on or before September 30th.
3 year olds must be 3 on or before September 30th.

ALL NEW ADMISSIONS WILL BE REQUIRED TO FILL OUT A FACTS TUITION FORM. TUITION IS DIVIDED OVER 10 MONTHS AND IS AUTOMATICALLY DEDUCTED FROM AN ACCOUNT OF YOUR CHOICE (SAVINGS OR CHECKING) ON EITHER THE 5th or 17th OF THE MONTH.

Tuition rates for the 2021-2022 are listed under Admissions. 

Note: For K-8 and Pre-School, registration and other fees are due regardless of date of enrollment.

Tuition Rates are determined by the Finance Committee based on information provided by the Diocesan Office. There is a Catholic and Non-Catholic tuition rate for students entering grades Kindergarten-8th grade. The definition of each category is:

Catholic: Parent/Guardian must be registered in a Catholic parish (it does NOT have to be Our Lady of Peace Parish) and the child must have a valid Baptismal certificate. There is also a second, third and each additional child discount.

Non-Catholic: Religion other than Catholic.

Greek Orthodox will be considered Non-Catholic. All Non-Catholic children must still participate in Religion classes and participate in Liturgical events. All Non-Catholic students participate in sacramental classes but are not eligible to make the sacrament.

Catholic and Non-Catholic rates DO NOT apply to 3 & 4 year old Preschool Tuition
